Marionette lines do not exist in isolation. They sit between the chin and the jowl, and how they read is shaped by what is happening above them (the volume in the mid-face), below them (the bone of the chin and jaw), and beside them (the muscle pulling the corner of the mouth down). One side usually behaves a little differently from the other, and the line on one cheek can be considerably deeper than the line on the other. All of that is part of the assessment.

Our approach is careful and honest. We look at what is really driving your concern. It might be loss of support at the bone, descent of the fat pads, an overactive muscle drawing the corner of the mouth down, looser skin, or a mix of these. Line Depth
& Definition

The most visible part of a marionette line is how deeply it reads at rest, when your face is relaxed rather than expressing. A line that is faint in conversation but deepens at rest tells a different story to one that disappears with movement. Assessment considers both states, the length the line runs, and whether the groove is etched into the skin itself or is the shadow of a fold sitting above it.

Mouth Corner
Position

The corners of the mouth can sit level, turn up slightly, or droop downward. As the depressor anguli oris becomes more active over time, the corners often turn down, which deepens the line beneath. Assessment considers corner position at rest and during expression.

Skin
Quality

The skin overlying the line carries its own signs: texture, fine crepe, pigmentation, and how the surface catches the light. These are assessed independently of the line itself.

Mentolabial
Groove

The crease between the lower lip and the chin (the mentolabial sulcus) often deepens alongside marionette lines, and the two readings are usually taken together. A deeper mentolabial groove changes how the chin reads in profile.

Jowl & Lower
Face Context

Marionette lines rarely arrive on their own. The jowl sits just outside them, the prejowl sulcus runs alongside, and the chin and jaw provide the structural backdrop. Assessment considers each of these in relation to the line itself, because reading the line without considering the architecture around it tends to underread the cause.

Symmetry

Most faces carry one line slightly deeper than the other. Assessment notes the difference and considers what it might be tracking back to.

Clinical Philosophy
The groove below the mouth corner no longer fades at rest. What reads as a deepening line is usually downstream: the bone of the jaw losing height, the fat compartment above deflating, and the muscle at the corner tightening. A good consultation reads each separately, and is honest about which can be helped without surgery.

Tissue Support & Lifting

Aptos Thread Lifts

Minimally invasive lifting using barbed threads to support and reposition the soft tissue of the lower face, including the jowl and tissue adjacent to the marionette line. Ultherapy

Marionette lines develop when several changes work together below the surface. The bone of the lower jaw gradually loses height with age, the fat compartments of the lower face deflate and descend, the skin loses collagen and elastin, and the small muscle that pulls the corner of the mouth down can become more active over time. Both genetics and lifestyle factors play a role. The contribution of each is assessed at consultation.

For many presentations, yes, though it depends on what is driving the line. In-clinic approaches may help soften how the line reads, support the surrounding tissue, and reduce muscle activity contributing to the appearance.

Marionette lines often begin to appear in the late thirties to mid forties, though both earlier and later onset is common and depends on genetics, sun exposure (particularly relevant for Brisbane patients given the Queensland climate), skin quality, weight changes, and underlying bone and fat anatomy. For some, only one side deepens first. The age at which lines first appear does not determine when, or whether, treatment is appropriate.

Results vary depending on the treatment type, individual metabolism, skin quality, and lifestyle factors. Some approaches offer effects measured in months, others in years, and many work best as part of a longer programme rather than a single session. Smile lines, also called nasolabial folds, run from the side of the nose down to the corner of the mouth. Marionette lines run from the corner of the mouth downward toward the chin and jaw. The two often deepen alongside one another and may share some underlying causes, but they are anatomically distinct and are assessed separately.

The corners of the mouth may turn down for several reasons, including activity in the depressor anguli oris muscle, loss of structural support from the chin and prejowl region, and skin laxity. In-clinic options may help soften how the corner reads, but expectations are realistic and outcomes depend on individual anatomy.
