The vermilion show reduces, the upper lip lengthens, vertical lines appear above the mouth, the philtral columns flatten, the cupid's bow effaces, and the corners of the mouth may turn downward. These shifts often compound existing structural concerns and are assessed as part of the overall mid and lower face picture.

The corners of the mouth influence whether the resting face reads as neutral, content, or slightly downturned. Assessed alongside the lips, philtrum, and chin as a whole.
The upper lip feels present but reads thinner than it once did. What reads as a less defined or flatter lip is usually three things together: bone support reducing at the maxilla, collagen thinning in the vermilion, and the soft tissue losing the fullness that once held the border clear. Aptos Thread Lifts
Minimally invasive lifting using barbed threads to support the tissue around the lips, including the oral commissures and the broader perioral region. May be discussed where the corners of the mouth, lower face support, or tissue position around the lips are part of the broader concern. Morpheus 8
Radiofrequency energy and microneedling are combined to stimulate collagen and tissue remodelling in the skin surrounding the lips. May be relevant for skin laxity, fine perioral lines, and texture concerns that frame the lower face. A dedicated consultation is required.

Lip volume and definition are influenced by three layers: the bone around the mouth, the soft tissue of the lip itself, and the skin and muscle surrounding it. With age, the bone gradually resorbs. The lip tissue loses volume and elasticity. The cupid's bow flattens, the philtrum lengthens, and the perioral skin thins and develops fine vertical lines. Sun exposure, smoking history, and genetics all influence how quickly these changes appear. Both structural and ageing factors are assessed during consultation.
