& Position

Where the high point of your cheek sits, and how far forward it comes, shapes the whole middle of your face and how the rest reads against it. A cheek that has lost volume or dropped can make the eyes look more tired, the smile lines deeper, and the jawline less defined. We look at how far the cheek comes forward over the bone, how full it is in front of and below the eye, and how it sits with the temple, eye and lower face.

The Fat
Underneath

The cheek is held up by several separate pockets of fat, and they empty at different speeds and in a different order. The deep ones under the cheekbone usually go first, well before the surface looks any different. We look at which pockets have lost volume, and in what order.

Your Cheek
Shape

Cheeks come in all shapes: high and angular, soft and round, broad and full. What suits a man's face and a woman's face is genuinely different. We look at the shape in three dimensions, against the rest of your face.

Where Under-Eye
Meets Cheek

The spot where the lower eyelid meets the cheek, often called the tear trough, is one of the first places midface change shows. As the cheek loses volume, this line lengthens and an under-eye hollow can appear, even though the eye itself has not changed. We treat this as one area, not two.

Changes
with Age

With age, slow changes in the bone, the deep and surface fat, and the skin quietly reshape the midface. The cheek can flatten, drop, and pull the tissue around it down too, leaving a flatter midface, a hollower under-eye, deeper smile lines and a heavier lower face. These changes often build on the cheeks you started with, so we look at them as part of the whole face.

From Cheek
to Jawline

When the cheek loses its support, the soft tissue that used to sit over the cheekbone slides down towards the jaw, feeding into smile lines, marionette lines and early jowling. We look at this alongside the cheek, smile lines and jawline as one picture.

Clinical Philosophy
From the side, the cheek looks flatter than it feels to the touch. That gap is usually three things at once: the cheekbone losing height, the fat beneath it deflating, and the tissue on top settling down with it. A good consultation reads each layer on its own, and is honest about what treatment can and cannot change.

Volume support rebuilds lost projection at the cheekbone and refills the deflated fat pockets in the midface. The areas we focus on may include the front of the cheek over the bone, and the outer cheek over the cheekbone. We also look at the area in front of and below the eye, where the deep fat sits. The aim is to put back the support that was there before, not to add volume beyond your own structure. This is only decided at a full consultation.

Bio-Stimulation

Collagen stimulation prompts your skin and the deeper tissue to make more of their own structural protein over the months after treatment. Where we focus depends on where the support underneath has weakened. The aim is to build support from within rather than lay volume on top, so the change comes on slowly and looks more like your own restored anatomy than added fullness. This is only decided at a full consultation.

Tissue Support & Lifting

Aptos Thread Lifts

A minimally invasive lift that uses fine barbed threads to gently move dropped midface tissue back up towards the cheekbone. Dr Asadi is an Official Trainer for Aptos Threads.

Radiofrequency & Microneedling

Morpheus8

Combines radiofrequency energy with microneedling to encourage the tissue to remodel and the skin to tighten across the midface. It can be worth discussing for skin laxity, texture and tissue quality, either alongside or after the volume-restoring options.

Surgical Care

Surgical Mid-Face Lift or Facelift

For more advanced midface drop, marked skin laxity, or cheek volume loss together with a lot of lower face change, a surgical opinion from a plastic surgeon may be the better next step. Surgical options include a mid-face lift, deep plane facelift and SMAS lift, all done in a surgical setting away from our clinic. Where this is the right path, Dr Asadi will refer you on rather than offer a treatment that will not do the job.

It depends on the approach. Some treatments cause only mild bruising or swelling that settles in a few days; others have a longer settling time of two to four weeks as the tissue settles and the final shape appears.
