Two things work together to create lines. The first is the muscles just under the skin that move every time you smile, frown, or concentrate. The second is the skin itself: the collagen and elastin that help it spring back, and the natural moisture that keeps it supple. A wrinkle assessment is really about working out which of these is behind each line, because a line driven by muscle movement needs a different approach to one that comes from the skin changing. Nearby concerns like smile lines form a different way again, more from lost volume, changes in the bone, and looser skin than from repeated expression, so we assess them on their own terms.
Most lines sit somewhere on a path from dynamic to static. Dynamic lines appear when you move your face and ease off when it relaxes. People usually know them by everyday names. There are crow's feet at the outer corners of the eyes, frown lines between the brows (sometimes called the 'elevens'), horizontal forehead lines, and bunny lines across the top of the nose when you smile. There are also fine lip lines around the mouth, which can show up whether or not someone has ever smoked. Static lines are the ones you can still see when your face is completely still. They are what dynamic lines slowly turn into after years of folding in the same spot. The clinical name for the muscle-driven kind is hyperkinetic lines, which simply means lines from a lot of muscle movement, and there is a wide, normal range in how expressive a face is.
Age adds its own changes on top. From the late twenties, the skin slowly makes less collagen each year, the elastin that gives it stretch starts to wear, its natural moisture and oils drop, and the fat pockets that cushion the face begin to flatten. So the skin recovers less and less from the same expressions you have always made. Bit by bit, dynamic lines settle into static ones, and new lines appear where the skin used to be smooth. This often goes hand in hand with facial volume loss elsewhere and with changes to the lower face. It is part of why so many people now ask about in-clinic options that look at the muscle and the skin together.
Men's and women's skin tends to age differently. Men's skin is usually thicker, with more collagen and more oil, so deeper lines often arrive later but can look stronger once they do. Frown lines and forehead lines are often more obvious on a man's face. Women's skin is thinner to begin with and changes faster around perimenopause, as it loses oestrogen support over a few years. Crow's feet and the fine lines around the eyes often show sooner, and they can respond differently to the same treatment.
